Family Screening Test: Is Someone I Love Struggling with Alcohol or Drugs? (Family CAGE-AID)

For: Family members, answering about the person they are worried about · Takes 1 minute · Reviewed by Dr. Pichayut Kasemphakdeephong (Psychiatrist), 6 September 2026

Family members are usually the first to notice that something has changed, but it is hard to know whether what you see is “just drinking a lot” or the start of a real problem. This screening test adapts CAGE-AID, an internationally used 4-question screen, so that a relative can answer on behalf of the person they are worried about, without waiting for that person to admit there is a problem or take a test themselves.

The result is a preliminary screen, not a diagnosis, but it helps families decide whether it is time to seek professional advice. Everything is calculated on your device. Your answers are not stored or sent anywhere.

Answer the following 4 questions with your family member or the person you are worried about in mind (referred to as "they" below), thinking about their drinking or drug use. Answer based on what you have actually observed or felt.
